?、贅鋱?jiān)果過敏是最常見的食物過敏之一。本研究中,我們?cè)谌巳褐性u(píng)估了它的致敏情況。
?、谠?004年至2014年期間,對(duì)36例患者進(jìn)行了回顧性評(píng)估:一些患者表現(xiàn)出食用榛子的癥狀,另一些患者則因?yàn)閷?duì)另一種堅(jiān)果過敏而避免食用榛子。診斷是通過皮膚點(diǎn)刺試驗(yàn)、全榛子提取物特異性IgE和Cor a1、Cor a8、Cor a9、Cor a14特異性IgE檢測(cè)進(jìn)行的,所有患者(除過敏反應(yīng)病例外)進(jìn)行口服榛子挑戰(zhàn)試驗(yàn)。③36例患者中有28例(78%)食用榛子出現(xiàn)癥狀,18例(64%)確診為榛子過敏。其中9例對(duì)Cor a 8(50%)敏感,2例對(duì)Cor a 1(11%)敏感,2例對(duì)Cor a 9(11%)敏感,2例對(duì)Cor a 14(11%)敏感。36名患者中有8人(22%)因?yàn)閷?duì)另一種堅(jiān)果過敏而不吃榛子。經(jīng)口服試驗(yàn)證實(shí)3例榛子過敏。這3人中有2人對(duì)Cor a 8(66%)敏感,而沒有一個(gè)對(duì)Cor a 1、Cor a 9或Cor a 14敏感。④ Cor a8(脂質(zhì)轉(zhuǎn)移蛋白)是西班牙兩組榛子過敏患的最重要的過敏原,兩組榛子過敏患者包括: 攝入榛子有癥狀的患者和因?qū)α硪环N堅(jiān)果過敏而規(guī)避攝入榛子的患者。

Component-Resolved Diagnosis in Hazelnut Allergy

Abstract:
Rationale
Tree nuts allergy is one of the most common food allergies. In our study we evaluate the sensitization profile in our population.
Methods
36 patients were retrospectively evaluated between 2004 and 2014: some presented symptoms eating hazelnuts and others avoided hazelnuts because were allergic to another nut. The diagnosis was made by prick-prick Specific-IgE to full hazelnut extract and Cor a 1 , Cor a 8 , Cor a 9 , Cor a 14 and in all patients ( except cases of anaphylaxis ) oral challenge with hazelnut was made.
Results
28 of 36 (78%)patients had symptoms eating hazelnuts and the diagnosis of hazelnut allergy was confirm in 18 patients (64%). 9 of these 18 were sensitizated to Cor a 8 (50%) , 2 of these 18 to Cor a 1 (11%) 2 of 18 to Cor a 9 (11%) and 2 of 18 to Cor a 14 (11%). 8 of 36 patients (22%) avoided hazelnuts because were allergic to another nut. Hazelnut allergy was confirmed in 3 cases by an oral challenge. 2 of these 3 were sensitizated to Cor a 8 (66%) and noone to Cor a 1, Cor a 9 or Cor a 14.
Conclusions
Cor a 8 (lipid transfer protein), was the most important allergen in our population of hazelnut allergic patients, in both groups, patients who had symptoms with the intake of hazelnut, and patients who avoided the intake because were allergic to another nut.
